As a consequence of the revision of personal data protection legislation that is the entry into force of Regulation of the European Parliament and the Council (EC) 2016/679 from 27.04.2016  (GDPR) taking effect from 25.05.2018 r. on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation), with respect to your privacy, we ask you to fill the following form and provide relevant consents enabling us to contact you.

In order to ensure that you are aware who and in which way is processing your personal data, we present below information which should help you to determine this.

1. The Personal Data Administrator is copmany Confirme Sp. z o.o. with the registered seat in Warsaw at Postepu str. 13/3
2. Personal data will be processed by us in order to run a base of prospective contractors, including your personal data, as contractor's representative, and establish cooperation with such contractors in the future or manage the consents related to the processing of personal data.
3. The basis for processing personal data is Art. 6 1(b) of the General Data Protection Regulation from 27 April 2016 GDPR, that is the consent of the concerned person, to process their personal data for one or more specified purposes. In this case, these are purposes specified in point 2.
4. Providing the personal information is voluntary but their lack may cause that the contractor will not be listed in the base of contractors.
5. You have the right to withdraw your consent at any time, which will have no effect on the lawfulness of the processing of personal data, which was performed based on your consent before withdrawing it.
6. You have the right to have access to your personal data, their correction and removal, and in addition to requesting limitation in processing and portability of such data.
7. You have also right to object to the processing of your personal data for direct marketing purposes in the extent in which the processing is related to such direct marketing.
8. The recipients of your personal data will be entities providing  on our behalf IT services, courier or postal services, transport services, legal services and subcontractors.
9. Personal data is kept until the withdrawal of said authorization, deciding to remove given contractor from the database or discontinuation of maintaining the base of contractors (depending on which of the specified circumstances occurs later).
10. Should you consider that processing personal data does not comply with GDPR provisions, you are entitled to make a complaint to the President of the Office for the Protection of Personal Data.
11. During the processing of your personal data there will be no automated decision-making, including profiling.
12. Your personal data will not be transmitted to third countries (beyond the European Economic Area) nor to international organisations.
